The forms themselves

Every PDF hosted on this site is sourced from official ACORD form documentation. We do not alter the form itself, its fields, or its structure.

Revision dates

Each form's revision date is checked against ACORD's own published revision history for that form. When ACORD issues an updated version, we replace the hosted PDF and update the listed revision date.

Reference and citation sources

Where a form page references outside material, for example a glossary term or an industry body's guidance, we link to that source directly on the relevant form page under "References." Examples include the National Association of Insurance Commissioners (NAIC) and, where applicable, state departments of insurance.

What we don't claim

We are not ACORD LLC, and we don't claim to be the authoritative source for ACORD's own form specifications. For the definitive version of any form or ACORD's own guidance, ACORD's own materials take precedence over anything on this site.

If you notice a form on this site doesn't match the current official version, please use our corrections policy to let us know.
